Requesting use of a facility
A Facilities Request Form
must be filled out in order to reserve a certain area of a facility, including events on the District calendar. Each school’s main office has forms on file. Please read the back of the packet and make sure you comply with all of the terms. After completing the form, please send it to the Principal at your school. Once the form has been submitted, it will be passed on to receive authorization from all coordinating administrators. If approved, you will receive the white copy of the packet. If there is a conflict, you will be notified.
Please submit your request at least 2 weeks prior to event.
If a form is not on file, that part of the building will be closed after school hours. This process is important in preventing conflicts, maintaining awareness of students’ whereabouts, helping to manage cleaning areas, and for the safety and welfare of students, staff and volunteers.
